SUBJECT : IMPLEMENTATION OF IMMUNIZATION PROGRAM IN OUR INSTITUTION

This is to inform you that our institution is now strictly implementing Immunization Program.
Vaccines are made available at our Pharmacy from the Provincial Health Office. Available
vaccines are the following:
 Diphtheria and Tetanus Vaccine Absorbed for Adults & Adolescents
 Hepatitis B Vaccine
 BCG Vaccines
Attached herewith is the Policy and Procedure regarding Immunization Program for your
information, guidance and compliance.
