Good Day Ma’am,

This is a letter of request for a transfer of residence from Acacia Residence Hall to Ipil
Residence Hall for the Midyear 2018-2019. I am currently in extended stay in Acacia Residence
Hall and have already obtained units for the midyear in CRS. My motivation for this request is
the support for my mental health, particularly my diagnosed Generalized Anxiety Disorder. The
presence of Ipil Residence Hall resident Patrick Angelo Narciso (Student Number # 2015-07643)
may be used to help me cope with this illness.
