31st Grand Prairie Cinco de Mayo Celebration

Booth Application for May 5, 2018
Once your application is approved we will contact you with more information. Every food vendor must
provide a fire extinguisher and table covers for his or her booth.
No Booth is allowed to sell any beverages (ex: water, lemonade, aguas frescas or soft drinks).

10 ft. by 10 ft. space, no tables or chairs provided. Electrical hookups will be available for your booth. (In order
to provide adequate power, please submit your application by the required deadline.) Committee provides tent
covers for all food booths (health and fire code rules) unless self-contained trailer.
***You must clean your area after the event; including sweeping and collecting your trash.***

 Food Booth (non-profit organization) $150
 Selling (non-food) booth $125

Food Vendors: You are required to attend a mandatory food handler’s training with Werner Rodriguez GP Environmental. You
must obtain a permit that costs $50.00 (cash or check). You MUST call The City of Grand Prairie Environmental Services Food
Handlers to make arrangements with them to take the class by April 13 (no exceptions). Grand Prairie Environment Service
Department is 972-237-8055. If you will be claiming a non-profit status, please provide your non-profit documents to the city and
submit copy with your application. Also, your booth will be examined by 10 AM in the morning of the event and you must pass
Environmental Services inspection in order to operate.
* The Grand Prairie Cinco de Mayo Committee reserves the right to accept or reject any booth application. You will receive
a confirmation, along with setup information. Any changes/misrepresentations will forfeit applicant’s participation in event
and you will be asked to leave the grounds.
