Bill of Lading Instructions

Print this bill of lading (BOL) and hand it to the driver at pickup. Failure to use a BOL will result in additional
charges and transport delays.

Prepare Your Shipment

Ensure your shipment is properly packaged with all items secure and protected to avoid possible damage. It
will be cross-docked with a forklift multiple times during transport. If you’re shipping an engine or other
mechanical goods, we recommend enclosing the shipment in a crate.
Document Your Shipment
Take pictures of your shipment prior to pickup in case there are damages during transit. We recommend also
keeping a copy of your spec sheet, or a picture of your shipment on a scale, to help out in the event of a
weight or dimension discrepancy.

Review BOL Information

• Add any special identification numbers to the Additional Services field.
• Add notes if a pickup service (example: liftgate service) has been paid for but not rendered.
• Confirm addresses and contact information.
• Provide an itemized list of your shipments.
• Sign and date.
Delivery Receipt
At delivery, your driver will have a delivery receipt (DR) for your consignee to review and sign. If there are
services listed on the DR but not rendered at delivery, it’s important to have your consignee note on the DR
that these services were not performed. Your consignee is also responsible for noting any damages to the
shipment on the DR prior to signing.

Pickup/Delivery Information
Contact your carrier using the information provided below. When you call, please ask for Dispatching. They’ll
be able to provide you with the most up-to-date status of your shipment. You’ll be asked to provide either
the pickup and delivery location, or the pickup number, which can be found in your booking confirmation
email.

shipper, or freight broker under 49 U.S.C. 13102, is not their agent and has no control over performance hereunder, and is not involved in a joint venture relationship with the parties. The parties agree that uShip will be fully
indemnified and held harmless for all fees, expenses, and costs incurred by uShip arising from or related to the fault, negligence or misconduct of the parties.
