Warmest greetings
We would like to ask for your permission to allow us to conduct an interview among the
staffs of your laboratory. This is in view of our project in PMLS 1 or Principles in Medical
Laboratory Science 1 entitled “Comparison of Primary and Secondary Category
Laboratories”. We are conducting this interview among the laboratories in Polomolok,
South Cotabato and General Santos City.
The interview would last only 15-20 minutes and would be arranged at a time
convenient for the staffs(e.g during breaks). Participation in the interview is entirely
voluntary and there are no known anticipated risks in participation on this interview. All
information provided will be kept in utmost confidentiality and would be used only for
academic purposes.
