Explanation of Owner Regarding Her Error in filing another SSS ID No. to branch
store and cancellation of the said SSS ID No.

I Julita Y. Tan of legal age residing at 201 Roxas Blvd. Tambo Parañaque City after
duly sworn in accordance with the law do hereby depose & sayeth that:

1.) That I am the Proprietor/Owner of the Generics Pharmacy – Main (formerly

JYT Pharmacy) located at 366 Quirino Ave. Baclaran Parañaque City with SSS
ID No. 03-9206827-1 and my branch store located at 37 Mata St. Andrews Ave.
VAB Pasay City with SSS ID No. 03-9231388-7.

2.) I am not aware that when you opened another business using the same
business name you should used only one SSS ID No. with the Main Store for
this unfamiliarity with the SSS rulings I do registered my branch store having
another SSS ID number registration which is not supposed to have new SSS ID
number for the branch store.

3.) For this error in filing that I have done may we request your office to please
cancel my branch SSS ID No. 03-9231388-7 & consolidate my branch
contribution & filing of R3 with our Main Office with SSS ID No. 03-9206827-1.
