first date of entry into the U.S.
___ I am a noncitizen who entered the U.S. as lawful nonimmigrant status and remain in the U.S. under the terms of
that visa until after age 26. Attach copies of immigration documentation showing first date of entry into the U.S.
Category D:
If you do not fall into one of the categories above or can not provide the requested documentation, you must submit a
“Status Information Letter” described in Category A or B.

of the above.

directly. This office will not call Selective Service. If you have already been awarded aid, no further disbursements will be
