AFFIDAVIT OF LOSS

address at Blk 3, Lot 1, New Paradise Molave Extension Nagpayong 2, Brgy.
Pinagbuhatan, Pasig City, after having been duly sworn in accordance with law, do
hereby depose and state: THAT–

1. I am a holder of COMPANY ID issued by IPM CDC WASTE

MANAGEMNENT “ PASAY PROJECT SITE” ;
2. On October 2, 2019, I noticed that the same was missing presumably
misplaced and nowhere to be found;
3. We diligently looked the whereabouts of my ID at the place where it was lost
but the same resulted in vain. We also searched it to our house but the same
can no longer be found.
4. I am executing this affidavit to attest to the veracity of the foregoing facts and
for whatever legal purpose it may serve.
