NOTES:

Keep with you and SECURE IN A SEPARATE FOLDER WITH PLASTIC ENVELOP THE ORIGINAL COPY OF
EACH OF YOUR APPLICATION CREDENTIALS.

Bring it with you only during the actual Paper Evaluation, the Interview, or WHENEVER REQUIRED.
Never ever consent in leaving your "original copies" with the school or DCS of choice without the proper
and official ACKNOWLEDGEMENT RECEIPT. Since the submission of the original copies of the teacher-
applicants' credentials IS NEVER REQUIRED UNDER ANY DEPED ORDER.

Try thinking.

What if mawala o ma-misplace ng staff sa school or DCS of choice mo yung mga original copies ng
documents mo o mapasok sila ng magnanakaw, or worst, masunugan... PAPANO NA ANG MGA ORIGINAL
COPIES NG APPLICATION CREDENTIALS MO? Are they going to secure all of it for you, or at least pay you
for damages so you can apply and secure (again!) for the 2nd copy of EACH and ALL of your application
credentials? Kaya isip-isip din. As no one else will do it for you during this phase.

(2) Secure a copy of the 2017 version of the Personal Data Sheet (PDS) from the CSC website or from the
TRTU Files. Print two (2) copies of it in LEGAL-sized bond paper. Front pages only. Fill out all 4 pages of
the PDS in your own hand writing using BLACK PERMANENT INK (no other color please!). Paste a
PASSPORT SIZE recently-taken colored PICTURE with white background and upper garment with colar.

Note: Be sure to wear light make-up and well groomed hair. Male applicants, no earrings or tattoo
showing please.

(3) Secure a Community Residence Certificate Tax from your respective City Treasurer's Office. Affix its
details onto your PDF and later onto your Omnibus Certification.

(4) Fasten your paper credentials in a LONG FOLDER with PLASTIC COVER according to the CRITERIA OF
RANKING, as provided in the DepEd Order No. 7, s. 2015, as follows:

A. Education;

B. PRC License(s) and other PRC credentials;

C. Teaching or Industry Experience(s) thru COEs or Service Record;

D. Specialized Training (related to your college degree and/or to the subject applied for);

E. Awards, commendations, publications (if any); and

F. Other documents.

(5) Prepare an APPLICATION LETTER and fastened it on top of all your paper credentials in your
APPLICATION FOLDER. See a sample copy of it in our TRTU Files.

Note: Photocopy it as part of your RECEIVING COPY. So no one can later claim that you did not submit
your application or the same was not received.

(6) Prepare your OMNIBUS CERTIFICATION and list down in it EACH AND ALL OF YOUR PAPER
CREDENTIALS - the ones you are submitting. See a sample copy of it in our TRTU Files.

(7) Bring your Omnibus Certification to a Lawyer Notary Public and have it notarized there. The fee is
usually, P100 - 200 per page. It is FREE though at the Public Attorney's Office or PAO.

(8) Photocopy your Notarized Omnibus Certification, as this has to form part of your Receiving Copy too.
So no one can later claim that you did not submit this and that document.

Note: Bring with you the originals for it might be checked before your Omnibus Certification is notarized.
Place it as the LAST DOCUMENT in your FOLDER.

(9) Buy a WHITE FOLDER WITH PLASTIC COVER, or as instructed in your respective Division's Recruitment
Memo.

(10) Fasten all your documents with a PLASTIC FASTENER.

(11) Place a TAB per document. (See attach sample)

(12) Write a 1-page TABLE OF CONTENTS and fasten it onto the INSIDE FRONT COVER of your Application
Folder.
(13) Print a FRONT COVER and attach it onto the front of your APPLICATION FOLDER to indicate on what
that folder is, and what it contains. It could be something like this... (just center it)

(14) Submit your APPLICATION FOLDER to the SCHOOL OF YOUR CHOICE and respectfully ASK the person
receiving your folder to sign in your RECEIVING COPY with his/her complete name, date, office telephone
number. Ask also on WHEN can you follow up on your application.

Note 1: DO NOT FORGET TO ASK THE FOLLOWING:

A. The DATE OF THE SCHOOL PRINCIPAL's INTERVIEW;

B. The DATE OF THE DEMO TEACHING. What to bring for the actual Demo Teaching. Will it be an on-
the-spot DT or not? ASK IN ORDER TO KNOW.

C. WHO WILL CHOOSE THE TOPIC for the Demo Teaching? Although with the release of the DepEd
Order No. 42, s. 2016 that requires the use of DLP in all Demo Teaching, it would be MORE
CHALLENGING (and quite unfair) to require any teacher-applicant to prepare an on-the-spot DLP and
deliver it in an hour with instructional materials, student-centered activities, assessment activities,
evaluation sheets, etc. Nonetheless, ask in order to be sure and more prepared.

Note 2: As much as possible, opt for perfect timing in filing your application with the OFFICE OF THE
SCHOOL PRINCIPAL. Do it after lunch or immediately after the school dismissal of students (BEFORE 5PM)
or when you think and see that he/she is most relaxed. Nothing beats a perfect timing.

(15) MEDITATE and FERVENTLY PRAY for God to bless all of your efforts with SUCCESS in accordance with
His Divine Plan to prosper you and never to harm you.

(16) While waiting for feedback/updates on your application, START WORKING on your Detailed Lesson
Plan (DLP) for your DEMONSTRATING TEACHING. As DLP may take some time to prepare, especially for
FIRST TIME DLP WRITERS.

PARA HINDI YUNG 1-2 DAYS AY DEMO TEACHING MO NA TAPOS NI HINDI MO PA ALAM ANG MGA
TOPICS UNDER THE GRADE LEVEL AND SUBJECT NA INA-APLAYAN MONG ITURO AT I-DEMO.
And worse, 1 day before your scheduled DT ay doon ka pa lang magpo post sa TRTU Wall at literal na
magmamaka-awa at manghihingi (manghihiram daw!) ng lesson plan sa mga kapwa guro mo para sa DT
mo.

BE A PROFESSIONAL TEACHER !!!

ALAM MONG NAG-A-APPLY KA BILANG ISANG GURO, PWES, PANGATAWANAN MO NA KAYA MO ANG
TRABAHO AT RESPONSIBILIDAD NG PAGIGING ISANG GURO.

AT BASIC SA PAGIGING ISANG GURO YUNG MARUNONG KANG GUMAWA NG LESSON PLAN, DLP O SEMI-
DLP O DLL PA YAN. KUNG HINDI KA MARUNONG PWES, PAG ARALAN MO MUNA BAGO KA MAG APPLY.

DAPAT ONE (1) WEEK BEFORE NG SKED NG DEMO TEACHING MO, WRITTEN AND PRINTED OUT NA ANG
DLP MO.

This way, you can still have some ample time to do the following:

A. POLISH on your chosen DLP sequence delivery - 4As, 7Es, etc. ba ang napili mong gamitin for your
CHOSEN TOPIC AND CONTENTS within your desired grade level and subject.

Notes: You may expected to refer to the prescribed K+12 CURRICULUM GUIDE of the DepEd for the
REQUIRED COMPETENCIES, BENCHMARKS, etc. See to it that your DLP suits and hits exactly on WHAT
THE STUDENTS NEEDS TO LEARN FROM YOU. This way, you will be realizing your TEACHING GOALS and
the STUDENTS' LEARNING OBJECTIVES.

You may also check out other open sources online on the same topics and contents here and abroad - for
a WIDER PERSPECTIVE, MOTIVATIONAL TECHNIQUES, TEACHING STRATEGIES, DELIVERY TACTICS of your
lesson;
B. Plan very well for your STUDENT-CENTERED DETAILED LESSON PLAN, as required by the DepEd
Order No. 42, s. 2016;

C. Prepare a MULTI-MEDIA PRESENTATION of your DLP; and

D. Prepare and print several copies of your worksheets;

E. Prepare other things you might need for BELL RINGER, MOTIVATION, CLASSROOM MANAGEMENT,
INSTRUCTIONAL VISUAL AIDS (aside from multi media computer presentation), etc.;

F. REHEARSE repeatedly YOUR actual delivery of your DLP before a mirror or teacher-
friends/classmates for their sincere and professional critique. This way, you may learn to edit and
improve on your DLP thus, gaining "mastery", and obtaining the much- needed "self confidence" in its
actual delivery;

G. TIME your actual delivery as stated in your DLP.

You may see the TRTU FILES or PHOTOS for the template of the DLP or refer to the Annexes as attached
in the DepEd Order No. 42, s. 2016 which you can find at the website of the DepEd. (www.DepEd.gov.ph)

Note: Please browse thru the TRTU Wall for the criteria in assessing DLPs during the actual Demo
Teaching. That way, you will be more mindful on how to do yours, in order to score the highest mark
possible per criterion. Be sure to include ICT in your DLP. It scores high usually with the right delivery.

Best wishes on your teacher- application with your DCS or DO of choice. - Gie Serrano
