NG/OG Tubes Placement- NICU


DEFINITIONS
A gastric tube is a tube passed into the stomach via the nose or mouth as a means of meeting
a patient’s nutritional needs, administering medications, rehydration and decompression
of the stomach.
Gastric tubes are short or long term devices that can come with or without a guide-wire.
The manufacturer’s recommendations must be followed to ascertain maximum dwell times
and specific instructions regarding insertion.

INDICATIONS

 Prematurity
 Feeding immaturity
 Poor oro-motor skills
 Infants receiving assisted ventilation

Uses

 Enteral feeding
 Medication
 Assess gastric residual volume,content, Ph
 Vent or gravity drainage
 Decompression of stomach

Contraindications

 Anatomical abnormalities e.g. esophageal atresia

Complications of feeding tube placement may include:

 Apnea
 Bradycardia
 Mucosal irritation;Perforation
 Worsening of gastr-esophageal reflux

Procedure
1. Obtain a physician’s order for NG or OG insertion
2. Assemble all required equipment. ***Do not leave catheter under radiant warmer
light for ANY LENGTH of time, or it will become too soft for insertion.
3. Infant may be in prone position or head turned to right side.
4. Measure the length of tubing from the infant’s nose to the infant’s earlobe, then to the
tip of the xyphoid process mark the position of the tube.
5. Flex the newborn’s chin on his chest to facilitate passage. The tube may be lubricated
with sterile water. Insert tube to the distance measured through mouth or nares
pointing downward. ***** If resistance is met, discontinue procedure and notify
physician.
6. Check for tube placement by aspirating stomach contents, and by instilling 0.5 ccs of
air into tube while auscultating with stethoscope over the epigastric region.
7. Monitor the infant’s heart rate and color during procedure.
8. After desired placement has been reached, secure with tape (avoid occlusion of the
Nares)
9. Document size of the catheter used, time of insertion, infant’s toleration of procedure,
vital signs and amount of aspirate or residual obtained.

Secondly measure from the bottom of the ear lobe to the observed midpoint between the xiphoid process
and the umbilicus

Gently insert into one nostril and advance tube posteriorly aiming the tube parallel to nasal septum and
superior surface of hard palate
