SUBJECT : REQUEST VARIATION ORDER- HFE PHASE II BUILDING

This refers to the Construction of Home for Elderly Phase II Building at Ili Norte San
Juan La Union. The project was awarded to ECDI Construction and started the work
Nov. 12, 2018 with 154 calendar days to complete the project. The undersigned has a
regular monitoring and meeting with regards on the daily implementation especially
on addressing issues and concern arise during the construction. Prior to the start of the
project, we have reviewed on site together with the contractor all the details of the
technical drawing of Phase II Building and the Phase I building that was constructed
with the same contractor. The following are the findings during the review.

1. There are indicated design/details in Phase II building that are not feasible to
construct because the prerequisite structure was not indicated in Phase I building
design and not constructed during Phase I. this was affected the design in Phase
II.
2. Details of design in Phase II Building that are not pleasing to see (Inside Trellis
along open below).
3. Outlet connection drainage details was not provided in the plan of Phase I and
Phase II.
4. The front design was not included in the program of works of Phase II.

At present, the project is now 84% accomplished and it needs now to execute the
remaining works of 12% simultaneously with this propose variation.

The following are the propose Variation order without additional cost.

1. Modification of the left elevation rear part of the building: From continuous
roofing structure with 2.8m x 5.55m combination of masonry and glass window
enclosure to canopy type roofing with open walls using tubular steel materials
with same specification of roofing.
2. Removing Trellis structure at open below part of the building.
