AFFIDAVIT OF LOSS
1, NONITO E. MALLO, JR., of legal age, married, Filipino with office address at
Inland Corporation Container Yard located at R-10 Southbound Bgy 105, Tondo, Manila,
after being duly swom to in accordance with law depose and say that:
|. 1am presently the Container Yard (CY) Supervisor of all of the Container Yards
of Inland Corporation (‘Company’) and have been occupying the said position
since May16, 2016;
Prior to my appointment as such CY Supervisor, | was the Team Leader of
Inland Corporation ("Company") assigned at Nestle factory in Lipa City,
Batangas;
‘As CY Supervisor, | am tasked, among others, with the conduct of inventory of
all containers and chassis stored in all of the Company's Container Yards;
‘Sometime in May 2017, | conducted a physical inventory of all containers and
chassis in the Company's CY in R-10 Southbound Bgy 105, Tondo, Manila;
In the course of the conduct of such physical inventory, | discovered that the
following chassis were missing, to wit:
(a) Chassis No. 1095 with Plate No. TUA-381;
{b) Chassis No. 3208 with Plate No. TUF-334;
The above-described chassis belong to DELBROS, INC., from whom the
Company is leasing the same on a long-term basis;
Diligent efforts exerted to locate the above-mentioned missing chassis not only
in the Company's CY in R-10 Southbound Bgy 105, Tondo, Manila but also in
the rest of the Container Yards owned and operated by the Company; were all
in vain;
The Company continued its diligent efforts to find the missing chassis and on
‘or about December 11, 2017, one of the Company's personnel spotted one of
“the missing Chassis (Chassis No. 1095, with Plate No. TUA-381) in a certain

garage along Agora St, Barangay NBBS, Navotas City;
. The said personnel was able to identify the chassis because he was part of the
team that fabricated the under ride (a protective device required by the
Company's client, Nestle) that was attached to the chassis;
Thereafter, the Supervisor of the Chassis Leasing Section of the Company and
the undersigned conducted an investigation of the matter;
As part of the investigation, the said supervisor, a representative from the Anti-
Camap Unit of the Navotas Police Department, a Barangay Tanod of NBBS,
Navotas City and myself, went to the garage along Agora St., Bgy. NBBS,
Navotas City where the missing chassis was earlier spotted;
Upoh reaching the said garage, we were able to identify and confirm that the
chassis earlier spotted by the Company's personnel in the said garage was
indeed one of the missing chassis (Chassis No. 1095 with Plate No. TUA- 381);12.We noticed, however, that the Chassis No. and Plate No. were already
removed. Photo of the said discovered chassis is attached as Annexes “A”
&"B” hereof,
13,When we showed the above photo (Annexes “A" & to our client,
DELBROS, INC., the latter confirmed that the same is indeed one of their
missing chassis because of the identifying marks “N N 2434" welded in the said
chassis.
J am executing this affidavit to attest to the truth of the foregoing, to request the
Anti-Camap Unit of the Manila Police Department to conduct a thorough investigation on
the matter and to facilitate our recovery of the missing chassis and for whatever legal
purposes it may serve.
