I, ANGELO GRACELLANO, Filipino, of legal age, widower,

with residence of Brgy. Crossing, Calamba City, Laguna after having
sworn according to law hereby depose and say:

THAT, I bought a HONDA MOTORCYCLE Plate No. DD 18562

covered by CR 228307741 and OR 780619641, attached as Annex
“A” and “B” respectively;

THAT, with slow economy and my low income I decided to

attach a SIDE CAR on the above motorcycle to augment our daily
needs in life, which I bought from SANDRO GARROL, DEED OF
SALE OF SIDE CAR is attached as Annex “C”;

THAT I am executing this affidavit to attest to the truthfulness

of the above facts for any lawful purpose it may serve.
