To Whom it May Concern;

This is to inform that MS. JOANNA TRINIDAD, 26 years old of Lipa City, Batangas, submitted
herself for consultation at our clinic dated December 11, 2019 in reference to the
emotional disturbance she has been experiencing in her workplace.

As for provisional diagnostic impression, Ms. Trinidad is seen to have symptoms of stress-
related disorders that severely affect her socio-emotional and occupational functioning.

This letter is issued upon the request of Ms. Trinidad for whatever purpose this may serve
her.

Given this 7th day of January 2020 at Mental Health Wellness Center, Lipa City Batangas.
