I am__________, a student from Grade_____. I would like to request for a copy a Certificate of Grades,
including the grade from last school year (2018-2019) 1st and 2nd sem. Together with the grades of this
school year (2019-2020) for the 1st semester. This is needed as a requirement for our college application
