3 Items of evidence
DUO requires a number of documents for the recognition or temporary service. Please do not send in more information
than requested. DUO will contact you if more information is needed.

• Photocopy(ies) of your list of grades belonging to your education diploma(s) 1

• Photocopy(ies) of pedagogical training aimed at working in education

• For recognition only and if you have an original Certificate of Good Conduct for the profession of teacher, please
enclose this. The certificate must not be older than three months.2

• Evidence of nationality (copy passport or EU residence permit)

• Photocopy of your valid identity card (if you do not hold evidence as mentioned above)

1 When these documents are not drawn up in Dutch, English or German, you must enclose a certified translation. These
are translations made by a translator, certified in the Netherlands or in your own country, that bear the translator’s
stamp. The translation must be attached to the source document in the source language.
DUO does not process applications without translations.

2 If you do not have a Certificate of Good Conduct, we may ask you for it at a later time.

sible for issuing these documents. I declare that I completed this application truthfully and that the diploma(s) submitted
by me has/have been issued to me in the country of origin according to the local regulations.
