PARENTAL CONSENT

This is to allow my son/daughter/ward, __________________________________________, a student of

___________________________________________________________, and a bona fide member of National
Federation of Junior Philippine Institute of Accountants- National Capital Region (NFJPIA-
NCR), to participate in the Academic Conference Northto be held on November 21, 2019
(Thursday) from 8:00 AM – 6:00 PM at Multipurpose Hall 3, University of the East
Caloocan, Caloocan City.

In the said event, we will be orienting and making the students aware about the future
career waiting for us after graduation. We will be also conducting seminars that would
motivate and inspire them to have a plan in their early years. This also aims to widen their
network to every local chapter of the North District.

Having considered the benefits that my son/daughter/ward will derive from his/her
participation in the above activity and having the understanding that every precaution will
be observed and duly taken by the activity organizers/ officers to ensure his/her safety, I
shall not hold the organizers/officers/adviser and/or school responsible for any untoward
incident that may happen beyond their control.
