Dear Sir/Madam,

Sub: Request for cancellation of my duties as the presiding officer.

Acknowledge the receipt of the order of appointment as the presiding officer for the upcoming
Karnataka Assembly elections.

However, considering the facts that

1) I have just recovered from abdominal tuberculosis, treated by Dr. Suresh Shenoy, KMC (Contact
No9972724845) for any cross verification if required.

2) I am suffering from high diabetes mellitus and hypertension and on continuous medication and
monitoring for the same from Dr. Roshan. (Contact No: 9845138717 )

3) Due to my health conditions, I need assistance from family members over the night

4) I am just 7 months away from my retirement

I humbly request you to please exempt me from the duties of the presiding officer.

Deeply trust that you will consider my constraints and relieve me from election duties.
