Identifying information

The client’s name is H.B. he is 13 years, old and first-born child of two siblings.

He is studying in grade 1 after he left his previous school where h was studying in grade 4.

He belongs to Punjabi speaking family and follows Islamic religion.

Behavior during sessions

The client was observed in classroom, activity areas. His clothes were not clean,

hair was not combed, teeth were not brushed and shoelaces were not tied. The client was

secretive and reluctant to share information initially. He was shy, anxious, and restless.

However, he got much better in later session. He was responding to questions asked. He

sits in head down posture in beginning sessions, keeps on moving his hands, feet, and was

not able to sit straight. The orientation of time, place, and person of client was accurate.

The client has good visual and auditory perceptions. He followed the test instructions well,

however was reluctant to perform on tests, and was slow while following different

commands.
Psychological evaluation

The client’s visual motor index is 80-89 due to which he falls under the category

of below average.

On the intelligence test, the clients mental age was 7 years and 1 month, and falls

under the category of mildly deficient.

Tentative diagnosis

Specific learning disorder co morbid with 314.01 (F90.2)

Prognosis

The client’s prognosis is unfavorable, even though the client tries to work hard and

overcome his academic issues. However, lack of parent’s support, encouragement is

causing hindrance for him.
