Dear Ma’am,

I would like to thank your good office for this opportunity to request a modular class.
Recently, I have incurred absences to my regular classes due to the following reasons:

1. I am engaged in different organizations and government agencies that requires my

hands-on supervision and presence respectively; San Narciso Woman’s Club
(SNWC), Department of Trade and Industry (DTI), DOST, MSME’s.
2. I have a full-time business.

Since I am covered by the US Veterans’ Educational Assistance, Lyceum of Subic permits yours
truly, to take the modular class to finish my course. I am very determined to accomplish the
requirements in my studies, as well as perform my sworn responsibilities to my family and
community.
