Dear Dr. Milo,

Good day! I am Megan Celini J. dela Cruz, a clinical clerk rotating under anesthesiology from
November 24-30, 2018. I was not able to attend lectures last Monday, November 26, due to
severe body pain and headache that started on Sunday and worsened through Monday. I was not
able to go to the health service and secure a medical certificate but instead I decided to rest on
that said day. I unfortunately missed high yield anesthesiology lectures that I would really like to
make up to.
