AFFIDAVIT OF LOSS OF RECEIPT

I, JULIE N. DELARA, student at University of Rizal System Taytay Campus

presently taking up Bachelor of Elementary Education Major in Content Course, after

having been duly sworn to in accordance of with the law, hereby depose and state;

That I am taking care of requesting and receiving the documents of Decemie

Jumamoy that sometime around November 27, I noted that the receipt and form in order

to receive the Transcript of Records was missing. That to my knowledge, the receipt

was lost when I left it in the Publication Office of our Campus and the other staff writers

clean the office. That I look great pains to look for the receipt but to no avail. That I had

reported the loss of the said receipt to the Registrar Office of our campus.
