Important reminders

1. Your payments and benefits are in Philippine pesos.


2. Only basic premiums and top-up premiums are invested into your funds. Rider premiums are not invested.
3. Your living benefit is your account value.
4. Your death benefit (Type 2 Level Face) is higher of 200% of your face amount, plus 125% of all top-ups, less 125% of all partial
withdrawals or your account value. It will not be less than 500% of the basic premium, plus 125% of all top-up premiums paid, less 125%
of all withdrawals.
5. The value on Your benefits illustrated and Customize your benefits sections assume that:
a) All premiums, top-ups and withdrawals are made at the beginning of the year;
b) All premiums and top-ups are fully paid on time, and there were no premium holidays except for the amount shown under
Customize your benefits section;
c) You did not withdraw any other amount except for the amount shown under Customize your benefits section;
d) Your full withdrawal value is your living benefit less applicable withdrawal charges;
e) Your partial withdrawal values are taken out of the living benefits under Customize your benefits section. However,
withdrawal charges may apply and these are not yet reflected in the values shown.
6. Contract debt period: If your account value is not enough to pay for the monthly deductions during the first five years, we will delay
collecting them until your account value is enough. Your policy will not end and we will still pay the death benefit if the insured dies. This
will not apply if you miss paying your basic or rider premium, or if your total withdrawals are more than your total extra investments.
7. Premium holiday: You can choose to delay paying your premiums as long as your living benefit is enough to cover the monthly
deductions and rider premiums. However, your living and death benefits may be lower as a result and your policy may end earlier than
when you pay your premiums on time.
8. It is important that you understand the contents of this proposal, as it will become part of your policy once we have approved your
application.

of this proposal that show how the policy performs using the company's assumptions and based on the Insurance Commission's
guidelines on interest rates.
I understand that since the fund performance may vary, the values of my units are not guaranteed. It will depend on the actual
performance and the value of my policy could be less than how much I paid. The unit values of my policy are regularly published.
I understand that the recommended minimum payment period of 10 years may not be enough to continue my policy and I may need to
pay more premiums if the account value is not enough to pay the policy charges.
