ABSENCE tracking procedure

TEACHER

1. Ask student to complete the 'Absence Notice' form.

2. Email the form to students if they are off campus.

3. Decide if the absence is reasonable or not by ticking 'yes' or 'no'.

4. Sign and date at the bottom of the page.


5. Photocopy or keep original of any absence document from student and attach to the
'Absence Notice'.
6. Submit all documents to the office.

ADMIN STAFF

1. File approved 'Absent Notice' in client file.

2. For unapproved absences, to update the Melbourne Poly Unapproved Absence spreadsheet
and file 'Absent Notice' in client file.

N.B. If student calls, emails, or comes to reception, Admin staff to do the following:

1. Ask student to complete the 'Absence Notice' form.

2. Email the form to students if they are off campus.

3. Photocopy or keep original of any absence document from student and attach to the 'Absence
Notice'.

4. Give the Absence Notice form to the Program Coordinator for approval.

5. File approved 'Absent Notice' in client file.

6. For unapproved absences, to update the Melbourne Poly Unapproved Absence spreadsheet
and file 'Absent Notice' in client file.
