Respectfully submitted to ENGR. CEDRICK R. BUTAO, CHDO –

OIC, this City the herein Deed of Absolute Sale of awardee Ms. Elvira A.
Hamot of Block 7, Lot 5, Furia Subdivision, Barangay Libretad, Butuan City.

After thorough review, the requirements for the execution of a Deed

of Absolute Sale, as provided by the CHDO, have been completely complied
with by the awardee, as follows:

1. Letter of Request for the Deed of Absolute Sale;

2. Lot Award;
3. Valid ID of the Awardee (Xerox Copy);
4. Certification of Full Payment as issued by the City Treasurer;
5. Tax Declaration;
6. Approved Subdivision Plan from the DENR (Blueprint);
7. Title;
8. Inspection Report conducted by the CHDO; and
9. Certification from the CHDO that Elvira A. Hamot is the actual
occupant of the lot, as per ocular inspection report conducted last
August 16, 2017.

Anent to this, it is recommended that the Lot Award attached be a

Certified True Copy from your office. Moreover, the attached Deed of Absolute
Sale appears to be proper as to form and substance, and in accordance to laws,
good morals, public policies and public order.

Therefore, the CHDO is required to produce seven (7) copies of the

said Deed of Absolute Sale and shall indorse the same in this Office for
countersignature and for the subsequent indorsement to the City Mayor for his
approval, as per authority granted to him under SP Ordinance No. 2518-2004.
