Injury : Superficial burn injury on face and neck

What happened:
On August 28, 2019, in ‘C’ shift at around 11:45 pm, the IP, contract employee of M/s Abreast
Engineering Co., was working in CC1 Cooling chamber. After lancing jam in strand 3 cooling
chamber, he was breaking the slag with the help of lancing pipe. During this, oxygen valve
attached with the pipe opened and small flash took place. Due to this, lower face and neck portion
of IP got superficial burn injuries (2nd degree 5% burn). He was immediately sent to the east plant
first-aid centre where first-aid was administered and therefore, he was referred to TMH. From
Burn Care Unit, he was released on 14tth September, 2019 and has joined duty thereafter.

Preliminary Findings:
1. SOP for lancing in cooling chamber (LD1/PCL-CC1/04/11) exists.
2. IP was not wearing neck guard.
3. Exposure at face shows that visor might have been in up condition.
4. Person was experienced & trained on lancing job.
5. The IP was skill certified from JNTVTI.

Immediate action:
1. IP was immediately taken to EPFA from where he was referred to TMH.
2. Incident was communicated to all employees of Caster area.
3. PIR has been logged (PIR no-IN00077775).

Recommendations:

1. PPE awareness to be increased amongst the lancers and helpers across LD shops for
proper use of neck guard & visor during lancing.
2. Training on mitigation measures after burn injury to be provided to all lancers & helpers.
3. Cleaning of oil and grease to be mentioned in SOP for the lancing job and retraining on
revised SOP to be provided.
4. Company employee should ensure that area is free of flammable materials like oil and
grease before starting the lancing job
5. Safe length of the lancing pipe should be marked on the pipe to avoid manual judgment
(Refer Lancing standard for safe length as per the suitability of the job).
