JUDICIAL AFFIDAVIT

I, JACQUELINE ISHMAEL y DICEN, 33/W of Callejon Drive Guiwan, Zamboanga

City after having sworn to in accordance with law, do hereby depose and state.

The person examining me is (Investigator) PO2 Mark T. Garcia with office address at
Zamboanga City Police Station 5, Barangay Divisoria, Zamboanga City. The examination is
being held at the same place. I am answering the questions ask of me by the Investigator fully
conscious, that I do so under oath and I am also aware that I may face criminal liability for false
t e s t i m o n y o r p e r j u r y.

QUESTION AND ANSWER

Q: Will you please tell to the investigator your personal circumstances?

A: I am Jacqueline D. Ishmael, 33/w of Callejon Drive, Guiwan, this city, employed at
Department of Environment and Natural Resources (DENR) Community Environment and
Natural Resources Office (CENRO), Zamboanga City and my present position now is Forester I
under Conservation and Development Section.

Q: What is your usual work or function in your office?

A: I am the Geographic Information System (GIS) Focal Person in the office, my task is more on
the generation and or consolidation of maps using GIS software within the AOR of Zamboanga
City.

Q: Do you know why I am here with you in the hospital?

A: Yes sir, you are here to get my statement in connection with the shooting incident at Callejon
Drive, Guiwan, this city that happened last March 7, 2019 about 6:30 PM wherein I am one of
the victims and my husband, Mohammad Reza U. Ishmael (deceased).

Q: Do you recall on or about 6:30PM of March 7, 2019, where were you?

A: Yes sir, I and my husband were on our way home boarded in our motorcycle Honda CRF
color red with MV File number 0901-0316290 driven by my husband and while reaching inner
portion of Callejon Drive, Guiwan, this city which is more or less 70 meters away to our
residence, I heard gun shots from my behind and immediately I felt pain on my lower back and
left hand and I fell down from the motorcycle while my husband went to the direction of a
concrete wall there and then the gun man shot him several times with his hands up shouting “
hindi kame, hinde kame” which means “we are not the one, we are not the one” then my husband
fell on the ground. After which, the gun man pointed his gun at me. At that instance, I position
myself side view and cover my face with my arm and shouted for help. Moment later, I heard
several voices of people followed by the arrival of policemen and ambulance of which I was
brought to the Doctor’s Hospital for my medical treatment.

Q: What happened to you in the hospital?

A: I was subjected for medical operation wherein my right kidney was taken due to the gun shot
that caused damaged to my right kidney.

Q: What happened next?

A: That on March 9, 2019, my sister in-law told me that my husband died in the said incident
due to several gun shots in the different part of his body.

Q: Prior to the incident, did your husband say anything to you?

A: None sir, my husband did not say anything to me as far as I recall.
Q: What did your husband do in Barangay Sibulao?
A: As far as I know, my husband and other technical personnel of CENRO Zamboanga City
went to barangay Sibulao to conduct information drive to the residence thereof regarding
Environment and Natural Resources (ENR) laws, rules and regulations.

Q: Do you have anything to say, add, alter or change of your statement given?
A: No more sir.

I, PO2 Mark T. Garcia, 31/s, Filipino, a resident of Tumaga, Zamboanga City, a bonified
member of Philippine National Police presently assigned at Zamboanga City Police Office, PS5,
Divisoria, Zamboanga City designated as Investigator after being duly sworn and say;
1. I was the one who conducted the examination of complainant Jacqueline D. Ishmael at
the office of investigation section at the aforementioned Police Station.
2. I have faithfully recorded or caused to be recorded the questions I asked and the
corresponding answers that the complainant gave.
3. Neither I nor any other person then present or assisting her coached the complainant
regarding her answers.
