EMPLOYEE LOAN AGREEMENT

AND PROMISSORY NOTE

This Employee Loan Agreement and Promissory Note (the "Agreement") is made this ___
day of ______, 201__ by and between ABC INC. (the "Company") and MR./MS. ("Employee").

WHEREAS, the regular employee is MR./MS.

WHEREAS, Employee has requested a loan from the Company for personal reasons;

NOW THEREFORE, in consideration of the mutual promises and covenants contained

herein, the Company and Employee agree as follows:

LOAN & PAYMENT. On the date first written above, the Company shall loan to Employee the
sum of Two Hundred Thousand Pesos (PhP200,000.00) and payable within two (2) years by
withholding of the following payment schedule and amount commencing on August 2019 to and
continuing until July 2021 with the balance if any due on such date.
