residing at Sitio Balani, Dagupan City and Brgy. Tebeng, Dagupan City, after been sworn to an
oath in accordance with, do hereby depose and say that:

1. We personally know JAPHETH F. DE LA CRUZ, born on FEBRUARY 13, 1991 at

PANTAL DIST., DAGUPAN CITY and close associate and friend to his mother.

2. We personally know that JAPHETH F. DE LA CRUZ has always used his surname “DE
LA CRUZ” in all his dealings and is in fact known to his family and friends.

3. Mr. De la Cruz’s School records however, bears the surname DELA CRUZ (without a
space between “DE” and “LA”).

4. The full name JAPHETH F. DE LA CRUZ and JAPHETH F. DELA CRUZ refers to one
and the same person.

5. That this affidavit is made in compliance with any OFFICIAL REQUIREMENTS and we
attest to the veracity of all the statements contained therein.
