AFFIDAVIT OF LOSS OF MARRIAGE CONTRACT

I, FRANCIA ESTEVEZ VALENCIA, Filipino, of legal age, married

and resident of Barangay Mayapa, Calamba City, Laguna, after being sworn
in accordance with law, depose and say:

THAT sometime on June 20, 1962, my parents LOLITA SILO and

CESAR ESTEVEZ entered into a MARRIAGE CONTRACT at Pasig City;

THAT the original copy of said MARRIAGE CONTRACT was lost

inside our house and cannot be located despite diligent efforts, hence
considered lost;

THAT my mother died last September 2, 2002 at Las Pinas Medical

Center, Las Pinas City due to “CARDIAC EMPHYSEMA;”

THAT during the lifetime of my mother, she was a member of the

SSS, SSS No. 04-0436989-7, Xerox copy is attached and made an integral
part of this affidavit of loss;

THAT I am executing this affidavit to attest to the truthfulness of the

above-stated facts for the SOCIAL SECURITY SYSTEM grant of pension
to my father CESAR ESTEVEZ and for whatever lawful purpose it may
serve.
