SCHOLARSHIP QUALIFICATION REQUIREMENTS

FOR BACHELOR OF SECONDARY EDUCATION PROGRAM

1. He/ she must be a Filipino citizen.

2. Either of the parents must be an El Shaddai member.
3. He/she must be endorsed by the PFCC where he/she belongs.
4. He/she must be in good health.
5. He/she must have graduated from High School not later than 2013.
6. He/she must fulfill all terms and conditions of the scholarship.
a. No general weighted average lower than 83 (for incoming freshmen);

b. Enrolled full time/ regular load;

c. No grades lower than 83% in core courses (for continuing student);

d. No failing grades in general education courses (for continuing student);

e. Failure to meet grade requirements means forfeiture of scholarship.

If the scholar voluntarily withdraws from the scholarship for any
reason, he/she shall be made liable to pay the full amount of
scholarship fund used. Failure to pay means the scholar’s
credentials maybe withheld by CDW until such time as payment is
made.
7. He/she must be of good moral character

Admission requirements
 Admission fee: PhP400.00 (non-refundable and to be paid at the
Cashier, Accounting Department).
Documents to be submitted upon application:
 Duly accomplished application/student information form
 Certified true copy of the latest Grades (form 138/report card, TOR)
 Photocopy of the National Career Assessment Examination Result
(NCAE) if applicable
 Two (2) recent 2”x 2” pictures in color w/ white background with
applicant’s name written at the back.
 Original & Photocopy of PSA Authenticated Birth Certificate
 Letter of application or recommendation (from former teacher)
 Long brown envelope
