Dear Parents,

I would like to ask your permission to allow your child to conduct a survey among the
residences in San Rafael Rodriguez Rizal in the first week of February. This is in view of
their capstone project, entitled, “Effects of Extemporize Oxidane Filtration System
(EOFS) using organic materials in filtering river water”. They were conducting the
survey among the residents in San Rafael Rodriguez Rizal.

Your permission is being sought to have your child allowed to participate in this study.
The researchers choose the residence in San Rafael Rodriguez Rizal as the subject of the
study since they are the one located near in the river around San Rafael Rodriguez Rizal.
The main purpose of their study is to produce a product Extemporize Oxidane Filtration
System (EOFS) that will be useful as a source of clean water for the community.

to conduct this study/survey at San Rafael Rodriguez, Rizal
