WAIVER

I,_____________________________________________, allow my son/daughter

to join the 13th Annual Regional Convention this coming February 20-23, 2020 at
General Santos City. Having signed this, I release the Regional Federation of any
liabilities and responsibility of any happening during the duration of the event.
Lest worse comes to worst, the Regional Federation is not obliged for any
damages of any kind. This binds not only to the federation itself, but to its
individual officers and adviser as well.
