Aldo is a four years and three months old male who was last seen in our clinic on
September 25, 2015. He was assessed to have Pervasive Developmental Disorder not otherwise
specified. He was on Occupational therapy once a week, speech therapy session once a week,
and on regular school daily. He started attending regular nursery class since June 2015.

At home, Aldo had fleeting eye contact. He would put on t-shirt with minimal assistance.
He associated play, for example “Peppa pig”, on what he sees and observes. Aldo would interact
with adults and no longer be quick tempered when playing with other children. His mother reported
that he would brush his teeth without help. He would remove his shoes and socks without
assistance. Mother noted that he had awareness of people around him. He would follow
commands at home. He would now speak few sentences like, “Mom, can I borrow your cellphone
please?”. He would occasionally lead his mother’s hand from time to time when he wants
something. Mother noted that Aldo still talks and sings when he’s alone and is engaged on parallel
play. His tantrums became minimal.

At nursery school, the teacher noted that he was cooperative. He had satisfactory marks
and was not left on his lessons. Aldo preferred movement activities and would become restless
when sitting for a long time. He was interactive with other kids. He would usually look onto what
other kids are doing at school. He also participates and sings in class.

OBSERVATIONS (of informant and evaluator):


-He had fleeting eye contact upon entering the room
-He sat less than ten minutes
-He walks around the room during the interview
-He was talking to himself while playing
-He followed few commands like returning toys to the basket
-He can name all colors and some pictures
-His speech was understandable
-He was easily distracted with the cubes during the interview
-He interacts then moves away to play.
IMPRESSION: PERVASIVE DEVELOPMENTAL DISORDER NOT OTHERWISE SPECIFIED
RECOMMENDATIONS: Continue occupational and speech therapy
Encourage cooperative play
For enrollment in summer program
Parenteral advice given: continue providing structured schedule of
activities and daily routine, allow him to do things on his own with
guidance. Be consistent in disciplining him, explain things to him
concretely in terms he would understand, when tantrums arise,
offer alternative. Encourage more social interaction, continue
enforcing 2 hours of TV, computer and ipad daily. Read to him
daily, praise him for his efforts, use positive reinforcement
