Description It was found that crane loose gear was not marked with the individual numbering
as shown on the cargo gear register and certificates.

Root Cause Due to wear and tear of crane hooks and shackles, origin marker was faded

Corrective Action Re-marking of numbers according register certificates using dies.

Preventive Action Always Check the marking and keep it well clear put a mark again when faded.

Recommendation 2
Description It was noted that hatch sealing rubber were generally permanently indented
measurement show that the indent ws about 10mm on thickness of the seal of
(about 20 pct). The hatch access seals appeared to be about 50% permanently
Indented. It is recommended that the ship gives consideration to changing of all
hatch rubber seals at the forthcoming dry-dock limit.

Root Cause Due to excessive wearnes of the touchplate rubber packing was indented about
10 cm and almost reach the replacement limit.
Corrective Action Recommendation for replacement of rubber packing and touch plate at next dry
docking in Feb - Mar 2020

Apply grease to all touch plate

Description It is recommended that tank procedure can be improve. Person entering the enclosed
space should wear harness (not just have available). The company should consider
supply of some individual gas detectors. The company should consider providing a
Tri-pod arrangement for use in the rescue from enclosed space.

Description It is recommended that the emergency generator room door is mark with warning
for use of ear defender. It can be advantageous to have as spare set of ear defender
in the emergency generator space. It is also recommended that better PPE sign be
provided at the incinerator fire retardent apron and gloves as well as full face shield
should be available.
No available ear muffs and warning sign at the entrance area of the emergency
Root Cause generator. No PPE sign provided at Incinerator and PPE available ready for
immediate used.
Corrective Action Put Ear mufffs and sign at the entrance area of emergencyu generator. Put PPE
sign in the Incinerator space such as fire retardent apron gloves & face shield.

Preventive Action Always check the sign and PPE is stored in the respective designated area.
