Activities are not reported with their budget number reference and even the words of the action were
partially or wholly altered that creates complication to refer it with budget.
Therefore, the narrative and budget should be reported exactly with budget number of the project, full
narration of the action or activity head, and ought to compare the performance with planned number of
events or participants.

2. Ensuring the consistency and dependability of attendants’ authenticate signature at any


workshop/meetings is the core of due diligence for an organization. There were some inconsistent
signatures of attendees on some workshops and this should be corrected in the future plans.

3. For any inconvenience like inadequacy of budgets or overburdened personnel, activities should not be
aggregated during implementation. In whatsoever the need arises, consultations should be concluded
with the focal person or program manager.
