FIRST NAME

I, ELISE REYES CRUZ, of legal age, Filipino, and a resident of Blk 2,

Lot 6, Victors Valley, Tigatto, Davao City, after having been duly sworn to
in accordance with law, hereby declare that:

1. I am the petitioner seeking the change of first name in: my Certificate of Live
Birth
2. I was born on Decemcer 18, 1995 at Davao City, Davao Del Sur, Philippines;

3. The birth was recorded under registry number 1237654

4. The first name to be changed is from ELISE to ALISA

5. The grounds for filing this petition are the following:

a) I have habitually and continuously used ALISA and I am publicly known

in the community with that first name;
b) The first name causes confusion as my sister’s name is also ELISE
REYES CRUZ;
6. I submit the following documents to support this petition:

a. baptismal certificate
b. driver's license no 4567213r
c. Ateneo de Davao University diploma
d. police clearance
e. Notice and Certificate of Posting
f. Certified machine copy of the Official Receipt of the filing fee
g. An affidavit of publication from the Mindanao Star
h. copy of the Mindanao Star clippings

7. I have not filed any similar petition and that, to the best of my knowledge, no
other similar petition is pending with any LCRO, Court or Philippine Consulate;

8. I have no pending criminal, civil or administrative case in any court or any quasi-judicial
body.

9. I am filing this petition at the LCRO of Davao City, Davao del Sur, Philippines
