Good day!
I am Madz Alcoy- Bautista working as a part of the Madz and Dzhei Band hereby
acknowledge the agreement between the first party and the second party that the service
charge (see attached image) waives the out of town charge in Los Baños Laguna on July 7
2018.
Kindly be informed that the band will be requiring crew dinner for 7 pax and professional
grade sound system with at least three microphones, mixer and two monitor speakers.

Furthermore, the band will render the services outlined below plus 3 song accompaniments
with the couple’s guest performer.
