AFFIDAVIT OF CONSENT

I, XXXXXXXXXXXXX, 11 years old, Filipino Citizen with residence at Villa

1, Ground Floor, Unit 2, Appleone Banawa Heights, Cebu City, Philippines, sworn
to in accordance with law do hereby depose and state that:

1. I am the daughter of YYYYYYYYYYY and I was born on 14 June 2008;

2. That when I was born, my mother YYYYYYYYYYY was not married to my

biological father and that my biological father never recognized me as his
child;

3. That I have never met or seen my biological father ever since I was born;

4. That I am currently living together with my mother and her husband

BBBBBBBBBBBBBB in Villa 1, Ground Floor, Unit 2, Appleone Banawa
Heights, Cebu City, Philippines.

5. That I signify my written consent, freely, willingly and voluntarily to be

adopted by my mother and her husband, BBBBBBBBBBBBBBBBB.

6. I understand the contents of this Affidavit and every word herein has been
carefully explained to me such that I know for a fact the legal effects of this
document and the decree of adoption applied for jointly by my mother and
her husband, BBBBBBBBBBBB, to protect me, and I hope that the Court
will grant it the soonest time possible;

7. I hereby execute this affidavit to declare my intention and consent as

above given and as well as to the truth of the foregoing facts am executing
this Affidavit to attest to the veracity of the foregoing and for whatever
legal purpose this may serve best.
