Problem Statement:
To Improve completeness of details of
Job Request as well as implementation
of work.

Poor communication and coordination between end users and maintenance

team.

The signatories are not reviewing properly the completeness of information

listed on the form.

Supervisor/Head has equally important activities. Therefore, reviewing of listed

information is neglected.

Some end users are not following the instructions provided by the
Administrative Division regarding the proper filling up of forms due to lack of
information and uncertainty. The implementation of work is also affected due
to lacking of details and irregularity of schedules.
