I, Christian G. Isidro of legal age, hereby expressly state that:

1. I have chosen BLOCKCHAIN - UNIONBANK to take my On-the-Job Training (OJT) at,

which is of our own choosing, for 284 hours in partial fulfillment of the requirements for
the degree in Bachelor of Science in Computer Engineering;

2. I commit to secure and submit a duly signed and notarized Memorandum of Agreement
between <name of Host Training Establishment> and STI <Name of Campus> for the
conduct of the On-the-Job Training (OJT);

3. I am aware that I have to pay the amount of Two Thousand Five Hundred (Php 2500) as
training fee for the On-the-Job Training (OJT) at STI COLLEGE CALOOCAN, which fee is to
be paid to the latter;

4. I have read the rules and regulations set by the Host Training Establishment and commit
that I will abide by the said rules and regulations.

5. I fully and voluntarily waive my right to hold STI BLOCKCHAIN - UNIONBANK and/or any
of its officers, employees, or representatives responsible for any case of untoward
incident that may happen to me during the duration of my training.
