Greetings!

I would like to ask your good office to consider my request. The following are the
requirements that I still need to gather because my appointment date in PRC will be on
February 12, 2020 and February 13, 2020:

 Certified photocopy of PRC professional identification card or a PRC certification

showing the teacher’s name, LET rating, and other information recorded in the PRC
Office.
 Certified photocopy of ratings obtained in the Licensure Examination for Teachers
(LET)/Professional Board Examination for Teachers (PBET)

By February 14, 2020, I promise that I will submit the requirements that are mentioned above
to fulfill my Omnibus Certification.

As of now, enclosed are my proofs for my appointment in PRC for my Certificate of Rating,
Certificate of Passing, including my Panunumpa ng Propesyonal form and temporary
document for my rating obtained in the Licensure Examination for Teachers
(LET)/Professional Board Examination for Teachers (PBET).
