AFFIDAVIT

I, NOEL G. LABAYO, of legal age, Filipino, and a resident of No.

97 Fabella Street, Barangay Plainview, Mandaluyong City, Metro
Manila, after being duly sworn to in accordance with law, do hereby
depose and declare that:

1. I have been a resident of No. 97 Fabella Street, Barangay

Plainview, Mandaluyong City, Metro Manila since 03 June 2015 but has
been a resident of Mandaluyong City for more than five (5) years now.

2. Since July of 2016 up to present, Ramon B. Laureta has

been residing at my place at No. 97 Fabella Street, Barangay Plainview,
Mandaluyong City, Metro Manila.

3. I have known personally Ramon B. Laureta for such a very

long period of time since 1991.

4. I am executing this affidavit to attest to the truth of all the

foregoing and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature
