1. Old Students/Pupils
A. Form 138 or Report Card
B. Proper haircut for boys
2. Incoming Grade 1,Grade 7 and Transferee students
A. Passed the entrance Test
B. Report Card duly signed by the Principal of the school last attended.
C. Certificate of Good Moral Character and Certificate of Transfer
D. Photocopy of NSO/PSA Birth Certificate
E. Interview
F. Proper Haircut for boys
3. Uniform]
-All students are required to wear their prescribed uniform in all school days and on some
occasions that call for complete uniform.
1. BOYS
-Black long pants, (Grade 4-6 and Junior High school Students)
-Black short pants (Pre-School to Grade 3 Pupils)
-Blue short-sleeve polo shirt according to prescribed model with the school seal on the pocket.
-Neat military haircut
-Black leather shoes with white, plain, short socks.
2. GIRLS
-Blue dress with the prescribed shade and design and with school logo
-Black shoes (no high heels or boots) with white plain short socks not sports socks.
3. PE Uniform
-Its color is Navy blue with a lining of green color t-shirt with printed logo and Navy Blue jogging
pants to be worn during Physical Education Periods and Every Friday
4. ID Cards
-It should be worn at all times within the school premises.
-Tap the ID on the ID tapping Machine that is placed near the Office for the attendance
-Only the prescribed ID card with lace is allowed thus no extra decoartions should be inserted in
it. The school will provide your ID card at a minimal amount. Borrowing or exchanging of ID
cards is prohibioted
