COMPLAINT-AFFIDAVIT

I, ERYHLL REENA M. GUIMPATAN, of legal age, married, Filipino and a

resident of St. Francis St., Barangay 178, Maricaban, Pasay City, after having been duly
sworn to in accordance with law, hereby depose and state that:

1. I am the legal wife of Rolly Q. Guimpatan, the victim in this case. A copy of the
marriage certificate is hereunto attached and made as an integral part hereof as Annex
“A”.

2. On April 30, 2013, at around 9:30 in the evening, I was at home with my husband and
with my children. While I was upstairs, atop the staircase, and is currently calling for my
husband to go upstairs who was then seated at the lower stairs, Ronald Ocampo
suddenly appeared and repeatedly stabbed my husband in the neck using two (2) knives.

3. My husband was bended at that time he was being stabbed. Blood flow in an instance.

4. While my husband was being stabbed, I screamed until my husband fell to the ground
and then someone came, which happen to be his cousin who brought him to the
hospital.

5. As a consequence, my husband died instantaneously due to the inflicted stab wounds.

6. My husband and Ronald Ocampo are cousins but I have known the latter for one (1)
year and knowing no conflict nor any disagreement between them.

7. I am therefore executing this affidavit freely and voluntarily in support of my intent to

file a case for MURDER and/or the appropriate criminal case against Roland Ocampo,
who is of legal age and a resident of Sitio Arellano, Barangay Menlo, Pasay City.

IN WITNESS WHEREOF, I have hereunto set my hand this day of ___ at Pasay
