LOGISTICS Rate Confirmation Agreement for AMX LOGISTICS Rate shown includes any applicable fuel surcharges, pickup and delivery charges, loading and unloading, out of route, detention, storage, and/or all arbitrary charges etc. Deviation from these rates must be approved in writing and signed by both parties. Any authorized unloading will only be reimbursed with a valid unloading receipt. All drivers should call AMX LOGISTICS for dispatch, prior to arriving at the shipper's facility. A contact, number for the driver or a 24hr dispatch number must be provided for tracking purposes. For high priority shipments, Macrapoint Tracking may be required: fllure to enable Macropoint tracking will resuit in $500.00 fine. Check calls must be made daily by 10am EST or the carrier will be charged a penalty fee of $100 per day. If pickup or delivery times are missed, without prior notification to AMX LOGISTICS, carrier will be subject to penalty charges of $100 per day. Additional late delivery charges assessed by the consignee will also apply. In order to collect detention, the carrier must notify AMX LOGISTICS, within 2 hours after appointment time, of any delays at either the shipper ar consignee, Late pick-ups or late deliveries are not eligible for detention charges. If applicable, the arrival and departure times must be stamped and/or written on the B01. All refrigerated loads must be run on continuous unless otherwise stated. Individual load temperatures will be listed on the rate con. And temperature logs must be provided to validate temperatures. Drivers are responsible for al load counts. If driver is not granted access to the loading dock, they must call the broker to notify of shortages or overages immediately Any overages/shortages/damages, must be reported to AMX LOGISTICS as soon as unloading is complete. No praduct will be disposed of or donated without written consent from AMX LOGISTICS, To receive reimbursement, all lumper receipts must be submitted to AMX within 48 hours of delivery appointment. ifthe receipts are not turned within the 48 hour timeframe, no reimbursement will be given to the carrier. Any directions given to the carrier are for informational purposes only Carrier acknowiedges that they are solely responsible for compliance with all applicable HOS regulations, ‘as well as all other FMCSA regulations. No drivers are authorized to break seals under any circumstances. Seals must only be broken by the consignee's personnel, Leads delivered withaut the seals intact will resultin fines and potential claims.
