AFFIDAVIT OF NON-OPERATION

I, MONANILLA C. ASIS, of legal age, Filipino, married, and a resident of

Upper Jasaan, Jasaan, Misamis Oriental, after being duly sworn to an oath in
accordance with law, do hereby depose and state that:

1. I am the School Administrator of New Horizon and Light Learning

Center with present school address at Upper Jasaan, Jasaan, Misamis
Oriental;

2. The said pre school commenced its operation in 2004;

3. Sometime in 2007 as strong typhoon inundated the old school campus

then located at Nahalinan, Lower Jasaan, Jasaan, Misamis Oriental which
prompted school administration to stop its operation in 2007 until 2010
because financial constraints hinders the school to rehabilitate its modest
buildings.

4. In 2010, New Horizon and Light Learning Center re-opened its operation
to its present school site because rehabilitation of said building from its
former site renders it practically impossible.

5. I am executing this affidavit in order to declare the fact that our school
has had been non-operational from 2007 to 2010 for reasons stated above;

6. Finally, I am executing this affidavit in order to attest to veracity the

foregoing facts and for whatever legal purposes it may serve.
