SWORN AFFIDAVIT

I, MARQ SONTAS , Filipino citizen, of legal age, single, and a resident of Don Edmark Ave.,
Santos Village, Quezon City, after having been duly sworn in accordance with law, hereby depose and
say:

1. That on Oct 20, 2018, around 7 o’clock in the morning, I visited my friend Mando Rugas
in his residence at 666 P. Kamalasan St., Carmona, Makati City.

2. That Mando Rugas was alone in his residence and is unable to walk because of cast in his
right leg.

3. That I helped him for his activities of daily living for that day and left exactly at 10
o’clock in the evening after I helped Mando Rugas put himself to bed to sleep.

Further Affiant sayeth none.
