Good Day Sir. I’m Gira Villarta. May I get your permission for granting me a request on
enrolling my PRE-REQUISITE SUBJECT which is PURPOSIVE COMMUNICATION and
simultaneously enroll also my OJT this semester because I can’t take my OJT without finishing
the subject which is required for OJT. I really want to finish my studies and with your help sir I
can achieve it.

SUBJECTS:

OJT(On-The-Job-Training) 24 units + Purposive Communication (3 units)

= a total of 27 units

I am really hoping for your kind consideration and cooperation sir. I wish to achieve what I
couldn’t achieve before and that is finishing my studies even if it’s not a bachelor program. This

Good Day Sir/ma’am. I’m Gira Villarta, a student of University of Makati, here I authorize
JALAL B. MIRA-ATO to take care of my enrollment because I am currently at batanes province
due to my grandfather who passed away recently and I’m the one taking care of the
responsibilities here and I’m currently the one here who is taking care of my lola and later on
june 5 which is my flight home. I have a guardian which is my aunt ALMA VILLARTA-SANCHEZ.
She couldn’t come because she is too busy handling her work duties. And I live alone currently. I
am really sorry for not being present at my enrollment, that’s why I am entrusting JALAL MIRA-
ATO to take care of my enrollment.

Good Am Sir. I wish to get your permission to grant me a request on my enrollment. I need to enroll my
pre-requisite subject which is purposive communication because I couldn’t enroll my OJT. Please sir I
really need your permission for enrolling my subject sir. The reason why I still have a left over subject
was because on 1st Year 1st Semester the subject was currently full of slots and the registrar won’t let me
enroll, there was other sections but it includes sacrificing my major subjects because of conflict of
schedule. I couldn’t take it also on the following semesters because we have fully loaded units which 28
units is the maximum while we have 27 units already and that’s why I couldn’t enroll purposive
communication.

Subjects:

OJT on-the-job-training (24 units) + purposive communication (3 units)

= 27 units all in all

I am really hoping for your kind consideration and answer. I am willing to study and work hard on my
final year as a CCSE and UMAK student and am willing to pass my subjects. Thank you and Godbless sir

Good Day. I am ALMA VILLARTA-SANCHEZ (guardian and aunt of GIRA VILLARTA) here I
authorize JALAL B. MIRA-ATO to supervise or take care of the enrollment phase of my niece. I
currently can’t attend because I am busy and have too many things to take care of that’s why
I’m entrusting JALAL MIRA-ATO to supervise my niece’s enrollment. The reason why my niece
couldn’t be there because she is currently at our province which is BATANES because my late
Father passed away and He is the grandfather of GIRA VILLARTA.
