AFFIDAVIT OF CANCELLATION

I, LUCILA VALENCIA HERMOSA, of legal age, married, Filipino citizen, and a

resident of E. Villanueva St., Zone IV, Socorro, Oriental Mindoro, after having sworn to in
accordance with law, hereby depose and state:

1. That I am the registered owner of a business under the trade name TWO H3
TAILORING, located at Socorro, Oriental Mindoro;

2. That the abovementioned business was duly registered with the Department of Trade
and Industry (DTI) on 6 January 2016 and was issued Certificate of Registration No.
03841684;

3. That at present, I am causing the cancellation of the said registered business due to
management prerogative and change in line of business;

4. That as far as my personal knowledge as the registered owner, I attest to the fact that
TWO H3 TAILORING, has no standing financial obligation to any individual, public
or private corporation or entity;

5. That I am executing this affidavit to attest the truthfulness of the foregoing

declarations under penalties of perjury, to respectfully request the DTI to CANCEL
Certificate of Registration, No. 03841684, and for other legal purposes.
